Rectangular in form with a perforated projection on the upper edge, one side engraved with the demon Pazuzu in profile to the right, holding snakes in his outstretched hands, with a dog to his left and a scorpion to his right, a star and crescent above; the reverse divided into six horizontal registers, with cross-hatched triangles framing a pseudo inscription
