A folding card with green cover, in Dutch and English, No. 1200 from the Identity Department of the Transvaal Branch of the Geneva Red Cross Society, with the pencil name of J.F. Burger, 23 Jaar, Cederberg Commando, Residence Pretoria, and containing the instructions 'In case of bearer of this being killed or wounded, you are requested to send this card through the nearest Commanding Officer, or Responsible Official, to the Identity Department above mentioned'
