The sixteen-armed deity standing in pratyalidhasana on two prostrate figures over a double lotus base in yab-yum with his consort Nairatmya, with his arms radiating around him and his principal hands crossed in vajrahumkara mudra holding a bell and vajra, his eight heads surmounted by elaborate crowns molded with crested jewels, wearing a tiger-skin dhoti, beaded festoons, and skull-garland, his consort wearing a skull necklace, tiara, and beaded apron, two small attendants seated behind, inscribed around the base

The Newari inscription around the base may be translated as follows: "On the third day of the bright half of the month of Sravana in the year 681, this image of Saptadasatmak Hevajra was consacrated on the occasion of the first offering ceremony for the anniversary of the death of Sakya Jasim Raja. The donors are the sons of the deceased, Sakya Bhiksu Sri Guna and his brother Bhasimka, enabling the deceased, by the merit of this act, to attain the paradise of Sukhavati and enabling the donors to have a long life and good health. May this be well and for all time!"
