PRINTED ON PARCHMENT, elaborately engraved, blue paper seal of the United States at top left, bold heading at top "President of the United States of America," beneath which an American eagle and the "E Pluribus Unum" motto fly above billowing clouds, a large vignette of crossed flags, cannon and other military paraphernalia at the bottom, with small legend "Engraved by J.V.N and C.H. Throop, Washington City," accomplished in manuscript. President Lincoln and Secretary Stanton appoint Samuel Smith a Second Lieutenant.
