(10 7/8 x 8in.), contemporary limp vellum, spine ink-lettered, ties, upper inner joint broken, lacks y10 at end (presumably blank), edges dampstained throughout and some leaves at front and back badly stained and softened by damp, occasional crude repairs with old patches, traces of scotch tape repairs, other stains throughout.

Collation: a-x y . 183 leaves (of 184, lacks y10, presumably blank), with foliation and signatures. Types: c. 285G (title), 158G (headings, headlines, colophon), 98G. Double column. Large pictorial woodcut on title of SS. Cosmas and Damian beneath twin arches, one and two-line woodcut illustrations of instruments in the text.
