This letter is written from London, which Wagner visited from 7 to 29 May 1877 for a series of concerts at the Albert Hall, which he and Richter conducted in an attempt to raise money against the huge deficit from the first festival at Bayreuth the previous year. Schnappauf was Wagner's personal physician and an intimate of the Wagner household.
