A standard printed form used to appoint those of proven "Patriotism, Valour, Conduct and Fildeity," to serve as officers to "the Army of the United States, raised for the defence of American Liberty, and for repelling every hostile Invasion thereof." It was not uncommon for the President and Secretary to sign such appointments in blank, leaving the actual appointments to be specified by the ranking field officers.
