A note in the colophon indicates that the manuscript belonged to Abu Muhammad al-Hasan ibn Muhammad al-Mu'alim and that he gave it to his son Abi'l-Fadl Mas'ud in AH 705/1305-06 AH.

On illnesses and their cures, Persian manuscript on buff paper, with 320 ff. plus 2 later fly-leaves, each with 15ll. of black naskh, significant words either picked out in red or magnified, some marginal notes, generally in red, colophon dated 8 Rajab 655, black paper boards with brown leather spine, incomplete at beginning, trimmed, loose in binding, and some staining and repairs
