Quarto (280 x 215 mm). Complete with all black and white photographs, color pop-ups, and all items affixed (balloon perished, as always). Original boards with lenticular illustration on front cover, black cloth spine with silver lettering (hinges split; ephemeron tipped on rear endpaper); cloth folding box. Provenance: P.K. Thomajan (inscription from Warhol; typed letter, tipped on front endpaper).

FIRST EDITION, INSCRIBED BY WARHOL WITH DOODLES, INITIALED IN 4 PLACES THROUGHOUT THE BOOK, AND SIGNED ON THE FRONT COVER. TOGETHER WITH A SIGNED POLAROID BY THE ARTIST BRIGID BERLIN -- an important figure in Warhol's entourage. Warhol inscribed this copy to graphic designer P.K. Thomajan who wrote to Warhol suggesting a meeting ("have a spectacular idea for a super gimmick book"), and asking for his copy to be inscribed. Warhol also signed Thomajan's letter. Brigid Berlin's Polaroid reveals the double-page spread in Index (Book) that is invariably made impossible to view due to the plastic balloon located there having sealed the pages together. Index (Book) is "one of the most important and exuberant Pop art objects ever published... it is also ONE OF THE ULTIMATE PHOTOBOOKS-AS-OBJECTS.
