Rectangular, enamelled in translucent white over a wavy guilloché ground, each end applied with gold laurel-chased band, the cover centering the applied gold Cyrillic monogram 'SN' beneath a crown, with a diamond-set thumbpiece, the inside cover engraved with Russian inscription, 'For dear Nikolai Nikolaievich Sergeev from your grateful and capricious patient Baron P.A. Bilderling', marked inside cover and base
