The theme of the sleeping man has a specific meaning in medieval moralistic literature and art, representing the vice of Sloth. As Panofsky explains, 'The pillow suffices to indicate the sin of laziness, according to the proverb "Idling is the pillow of the devil"'. Drer's 'doctor' is sleeping the 'Sleep of the Unjust', as he has not earned his repose through work. Satan takes advantage of this easy target, blowing softly in the dreamer's ear, knowing that he will not be able to resist temptation.
