To Chakotay's people, the medicine wheel represented the universe, both inside one's mind, and outside. When one was near death, it was said that their soul had gotten lost on the wheel. When Chakotay was severely injured in "Cathexis" in Star Trek: Voyager, Torres used the wheel to help guide him back to the world of the living.
