This ijaza (certificate) was issued to Sheikh Muslih al-Din, son of the Dervish Sayyid Ibn 'Umar Pasha (whose genealogy is given all the way back to the Imam 'Ali). It is witnessed by a number of people with Turkish names and attributes, although it has not been possible to identify these.
